I just updated equo - I had just over 400 packages still to update.
Here's hoping!

I am trying to avoid my wife - it's her system I've baffed. I left a note on the monitor yesterday morning before work saying 'do not touch!' - that way I could ssh in through the vpn. I don't know why - but she UNPLUGGED THE NETWORK CABLE. She said she didn't touch the keyboard or mouse at least. The best part was when she complained that it wasn't fixed last night. :)

I've seen this error several times while trying to run update / upgrade while running the 5.1-r1 LiveCD (intending to upgrade then install). equo database check fails and equo update --force runs but doesn't fix the problem.
All this is after an initial "update" followed by a successful "equo install entropy". It is when I then try to run upgrade (117 packages) at the Updating database step that it dies with:
table packagesignatures has 4 columns but 5 values were supplied...

I see that the 5.1-r1_x86_K LiveCD copy of the packagesignatures table in equo.db has 4 columns (doesn't include the GPG sig). I also see that a system that has been running 5.0 since before 5.1 has a packagesignatures with 5 columns.

At what point does the file get converted? Is it done on the hard-drive install?

Did a hard drive install and saw equo announce:
updating repository metadata layout, please wait!
Dug into the source and I see that it depends upon:
if entropy.tools.islive(): # this works
if etpConst['systemroot']:
structure_update = True
else:
structure_update = True
So I guess etpConst['systemroot'] has to be set for the database to be updated in the LiveCD environment, but I can't understand why it wouldn't be set.

Should mention that the problem is easily repeatable by booting LiveCD, then "equo update", "equo install entropy", then "equo install <anything>" or "equo upgrade"...the key is to get entropy upgraded while still running on LiveCD.
