In entropy you have branches corresponding the releases. Starting with sabayon 3.5 there was branch '3'. Then sabayon 4.x came and used branch '4'. Strange, It is working here, from fresh installed and updated 4.2 KDE. I don't start it from the command line though, I start it by opening the KDE Menu -> typing 'pav' and choosing 'PulseAudio Volume Control' Maybe it is your Gtk Theme settings? The fglrx driver is tied closely to the version of kernel and version of X.Org software, so I wouldn't waste your time trying to get the Catalyst 9.3 or earlier versions working with the latest kernel and X.Org I got Catalyst 9.3 working wit kernel 2.6.29 without any problems for may ATI X300 card....

If you removed pulseaudio, don't forget to reconfigure your applications to use alsa mixing instead of pulseaudio. In smplayer you need to change the audio output from 'pulse' to 'alsa'. I indeed have a x86 install, but I'm still running Sabayon 3.5 with XFCE desktop environment. I will do a clean install of 4.2 KDE somewhere next week hopefully. Just thought of something. I start pulseaudio from one of the bootlevels (I used rc-update to add it), but everywhere I read it is recommended not to do it this way. If I understand it correctly, your 5.1 sound is working if the source contains all the channels. But if the source only contains 2 channels (front left and front right aka stereo) it doesn't duplicate the sound to the other channels (surround left, surround right, center and LFE) ?
